If you have dental or health insurance that may cover part of the implant procedure, bring your insurance card to verify your benefits and determine your financial responsibility.
A cosigner (If applicable)
If your treatment plan involves financing or payment arrangements, you may need a cosigner to complete the paperwork.
Previous exams or test results
